General Affidavits

Oaths/declarations
This category is for oaths and declaration forms. These forms typically do not carry court headings but require a person to make an oath in a form they are filling and get it commissioned as an Oath or Declaration.
All you need to do is upload the scanned form, which should already be signed and have your passport photograph in the appropriate column.
